How does it work? Maxolon high dose injection contains the active ingredient metoclopramide hydrochloride, which is a type of medicine called a dopamine antagonist. It is an anti-emetic medicine, meaning it is used to treat nausea and vomiting. Metoclopramide works primarily by blocking dopamine receptors found in an area of the brain known as the chemoreceptor trigger zone (CTZ). The CTZ is activated by nerve messages from the stomach when an irritant is present. It is also activated directly by agents circulating in the blood, for example anti-cancer medicines. Once activated, it sends messages to another area of the brain, the vomiting centre, which in turn sends messages to the gut, causing the vomiting reflex. Blocking the dopamine receptors in the CTZ prevents nausea messages from being sent to the vomiting centre. This reduces the sensation of nausea and prevents vomiting. Metoclopramide also acts in the upper end of the digestive system, where it enhances the action of a natural chemical called acetylcholine. Increasing the action of acetylcholine results in the tightening of the muscles at the entry to the stomach, relaxation of the muscles at the exit of the stomach and increased contraction of the muscles in the stomach itself. These actions speed the passage of food through the stomach into the intestine, which physically helps to prevent vomiting. Both actions make metoclopramide useful in treating nausea and vomiting due to many causes. This high dose preparation of metoclopramide is given by a drip into a vein (intravenous infusion) to treat nausea and vomiting caused by cancer chemotherapy.
Why is Reglan prescribed? Reglan increases the contractions of the stomach and small intestine, helping the passage of food. It is given to treat the symptoms of diabetic gastroparesis, a condition in which the stomach does not contract. These symptoms include vomiting, persistent fullness after meals, feeling of indigestion, heartburn, nausea, and appetite loss. Reglan is also used, for short periods, to treat heartburn in people with gastroesophageal reflux disorder (backflow of stomach contents into the esophagus). In addition, it is given to prevent nausea and vomiting caused by cancer chemotherapy and surgery.
Metoclopramide (Reglan) side effects If you experience any of the following serious side effects, stop taking Metoclopramide and seek emergency medical attention or contact your doctor immediately: - seizures or hallucinations.

- changes in vision;
- yellowing of the skin or eyes;
- depression;
- anxiety, difficulty breathing, jitteriness, agitation, or insomnia;
- uncontrollable movements or spasms of your arms, face, tongue, jaw, lips, legs, or other body part;
- an allergic reaction (difficulty breathing; or hives); swelling of your lips, tongue, or face; closing of your throat;
Other, less serious side effects may be more likely to occur. Continue to take Metoclopramide (Reglan) and talk to your doctor if you experience - increased frequency of urination.
- changes in your menstrual cycle; or
- breast tenderness or swelling;
- fluid retention (swelling of hands or legs, bloating);
- confusion;
- dizziness, drowsiness, or headache;
- nausea or diarrhea;

Recommended dosage ADULTS Symptoms of Gastroesophageal Reflux The usual dose is 10 milligrams to 15 milligrams of Reglan, up to 4 times a day, 30 minutes before each meal and at bedtime, depending upon the symptoms being treated and the effectiveness of the dose. Treatment usually lasts no longer than 12 weeks. If symptoms occur only intermittently or at specific times of the day, your doctor may give you a single dose of up to 20 milligrams as a preventive measure. Symptoms Associated with Diabetic Gastroparesis or Gastric Stasis The usual dose is 10 milligrams 30 minutes before each meal and at bedtime for 2 to 8 weeks. OLDER ADULTS Relief of Symptomatic Gastroesophageal Reflux Older adults may need only 5 milligrams per dose.
