How does it work? Anafranil capsules contain the active ingredient clomipramine, which is a type of medicine called a tricyclic antidepressant (TCA). This type of medicine acts on nerve cells in the brain. (NB. Clomipramine is also available without a brand name, ie as the generic medicine.)
In the brain there are numerous different chemical compounds called neurotransmitters. These act as chemical messengers between the nerve cells. Serotonin and noradrenaline are neurotransmitters and they have various functions that we know of.
When serotonin and noradrenaline are released from nerve cells in the brain they act to lighten mood. When they are reabsorbed into the nerve cells, they no longer have an effect on mood. It is thought that when depression occurs, there may be a decreased amount of serotonin and noradrenaline released from nerve cells in the brain.
Clomipramine works by preventing serotonin and noradrenaline from being reabsorbed back into the nerve cells in the brain. This helps prolong the mood lightening effect of any released noradrenaline and serotonin. In this way, clomipramine helps relieve depression.
Clomipramine also blocks receptors in other areas of the body, and this can cause side effects such as drowsiness. This means clomipramine may be particularly useful in treating depression in people who are also anxious and agitated.
It may take between two to four weeks for the benefits of this medicine to appear, so it is very important that you keep taking it, even if it doesn't seem to make much difference at first. If you feel your depression has got worse, or if you have any distressing thoughts or feelings in these first few weeks, then you should talk to your doctor.
Clomipramine may also be used in the treatment of obsessive disorders such as obsessive-compulsive disorder and phobia disorders. It is not fully understood how clomipramine works in these conditions but it is thought to be related to the increased effect of serotonin and noradrenaline in the brain. The first few days of clomipramine therapy for these disorders may involve an increase in anxiety and panic, however, this generally gets better within the first two weeks of treatment.
In addition, clomipramine may be used to treat cataplexy (sudden muscle weakness) associated with narcolepsy. Clomipramine is thought to be of use in this condition by increasing the stimulating effect of the chemical noradrenaline in the brain.
What should I watch for while taking clomipramine?
Visit your prescriber or health care professional for regular checks on your progress. It can take several days before you feel the full effect of clomipramine.
If you have been taking clomipramine regularly for some time, do not suddenly stop taking it. You must gradually reduce the dose or you may get severe side effects. Ask your prescriber or health care professional for advice. Even after you stop taking clomipramine it can still affect your body for several days.
You may get drowsy or dizzy. Do not drive, use machinery, or do anything that needs mental alertness until you know how clomipramine affects you. Do not stand or sit up quickly, especially if you are an older patient. This reduces the risk of dizzy or fainting spells. Alcohol may increase dizziness and drowsiness. Avoid alcoholic drinks.
Do not treat yourself for coughs, colds or allergies without asking your prescriber or health care professional for advice. Some ingredients can increase possible side effects.
Your mouth may get dry. Chewing sugarless gum or sucking hard candy, and drinking plenty of water will help.
Clomipramine may cause dry eyes and blurred vision. If you wear contact lenses you may feel some discomfort. Lubricating drops may help. See your ophthalmologist if the problem does not go away or is severe.
Clomipramine may make your skin more sensitive to the sun. Keep out of the sun, or wear protective clothing outdoors and use a sunscreen. Do not use sun lamps or sun tanning beds or booths.
Clomipramine can affect blood glucose (sugar) levels. If you are a diabetic, check your blood sugar more often than usual, especially during the first few weeks of clomipramine treatment. Call your prescriber or health care professional for advice if you notice a change in the results of blood or urine glucose tests.
If you are going to have surgery or will need an x-ray procedure that uses contrast agents, tell your prescriber or health care professional that you are taking this medicine.
Special warnings about Anafranil (Clomipramine) This drug should be used with caution in children with depression. In clinical studies, antidepressants increased the risk of suicidal thinking and behavior in children and adolescents with depression and other psychiatric disorders. Anyone considering the use of Anafranil (Clomipramine) or any other antidepressant in a child or adolescent must balance this risk with the clinical need. In children, Anafranil is only approved to treat obsessive-compulsive disorder. Additionally, the progression of major depression is associated with a worsening of symptoms and/or the emergence of suicidal thinking or behavior in both adults and children, whether or not they are taking antidepressants. Individuals being treated with Anafranil (Clomipramine) and their caregivers should watch for any change in symptoms or any new symptoms that appear suddenly--especially agitation, extreme hyperactivity, restlessness, panic, hostility, anxiety, and suicidal thinking or behavior--and report them to the doctor immediately. Be especially observant at the beginning of treatment or whenever there is a change in dose. If you have narrow-angle glaucoma (increased pressure in the eye) or are having difficulty urinating, Anafranil (Clomipramine) could make these conditions worse. Use Anafranil with caution if your kidney function is not normal. If you have a tumor of the adrenal gland, Anafranil (Clomipramine) could cause your blood pressure to rise suddenly and dangerously. Because Anafranil (Clomipramine) poses a possible risk of seizures, and because it may impair mental or physical ability to perform complicated tasks, your doctor will probably warn you to take special precautions if you need to drive a car, operate complicated machinery, or take part in activities such as swimming or climbing, in which suddenly losing consciousness could be dangerous. Note that your risk of seizures is increased: - If you are taking another medication that might predispose you to seizures
- If you have a history of brain damage or alcoholism
- If you have ever had a seizure
As with Tofranil, Elavil, and other tricyclic antidepressants, an overdose of Anafranil (Clomipramine) can be fatal. Do not be surprised if your doctor prescribes only a small quantity of Anafranil at a time. This is standard procedure to minimize the risk of overdose. Anafranil (Clomipramine) may cause your skin to become more sensitive to sunlight. Avoid prolonged exposure to sunlight. Before having any kind of surgery involving the use of general anesthesia, tell your doctor or dentist that you are taking Anafranil (Clomipramine). You may be advised to discontinue the drug temporarily. When it is time to stop taking Anafranil (Clomipramine), do not stop abruptly. Your doctor will have you taper off gradually to avoid withdrawal symptoms such as sleep problems, nausea, irritability or worsening emotional or mental problems, high fever, headache, general feeling of illness, fever, dizziness, vomiting.
